Click to read the story of how a patient came to us for non-healing chronic wounds and discovered 80 to 90% stenosis of his right internal carotid artery after carotid screening. As a result, he underwent TCAR procedure and currently healthy with no complaints.
This young 92 yr old man was first seen in July for over 6 months of non-healing wounds and lymphedema. The lymphedema (chronic swelling) contributed greatly to the wounds as the swelling causes the skin to stretch, hindering the healing process. Pt underwent wound care therapy at our center, in conjunction with compression wraps. Two weeks after starting compression, we saw a drastic decrease in wound. Within 9 weeks patient’s wound had completely healed! The patient was then advised to be on a lymphedema pump which has reduced swelling in his both legs to normal. He says he can see his ankles again!
We screened patient for Carotid Stenosis given his history which revealed 90% stenosis of his right internal carotid artery. Patient underwent a TCAR (Transcarotid Artery Revascularization a minimally invasive carotid stent perfect for someone his age) and now have less than 50% stenosis post procedure.

The internal carotid artery is one of the four main arteries that supplies blood to the brain. If any of those arteries are blocked or has plaque built up, the patients are at risk of stroke. We always have carotid screening done based on patient’s age or comorbidities such as diabetes, smoking, family history of stenosis. Generally patients are asymptomatic, hence the danger!
